HOW DO I SIGN UP FOR A MEMBERSHIP?
Step 1: Have the Head of Household complete the attached application
- Community Member Application
- Silver Sneaker / Silver & Fit Application
- JBU Family & Retiree Application
Step 2: Bring the following to the WLHC Front Desk during membership services hours for processing:
- Completed application (see above)
- Form of payment
- Form of ID for address verification*
*Each adult must present identification verifying that they reside at the same address as the Head of Household (i.e. driver’s license, utility bill, etc.) in order to be eligible for a household membership.
NOTE: The membership can be activated by the head of household, however, all other adults and dependents age 14 years and older must get a picture ID taken prior to using the facility for the first time.
Step 3: Begin using and enjoying the great benefits of your WLHC membership!
