Required Health Forms and Immunization Records

Immunization Requirements

The Arkansas Department of Health requires John Brown University to document that students who plan to take face-to-face classes on JBU's main campus or regional locations have met the state’s immunization requirements. In order to be compliant, JBU must have a record of MMR immunizations.

Required Immunizations: 2 doses of MMR (measles, mumps, rubella)
Suggested Immunizations: Meningitis, Tdap (tetanus, diphtheria, pertussis)

JBU accepts four types of records with the dates in which the immunizations were received:

  1. A doctor’s record signed by the physician
  2. School immunization records
  3. Military immunization records
  4. A record from the state health department**.

Health Form

Traditional/Residential Undergraduate students are required to complete the Health Form, located within the the Pre-Registration Paperwork. Completing this form is also required in order to register for classes whether the student registers during Early Registration or at New Student Orientation.

NOTE: Online Undergraduate and Graduate students are NOT required to complete the Health Form.

Immunization Records

Students are required to submit immunization records from a health care provider to John Brown University before they may begin classes. A student's deadline for submitting immunization requirements are as follows:

  • Residential/Traditional Undergraduate students taking classes on JBU's main campus in Siloam Springs: Immunization records can be submitted up to 30 days after the first day of the face-to-face class. However, it is preferable that students submit their immunization records during the registration period.
  • Graduate and Online Undergraduate students who plan to take face-to-face classes on JBU's main campus or regional locationsImmunization records can be submitted up to 30 days after the first day of the face-to-face class.

There are possible exemptions to having to present a record of these immunizations. Those born prior to 1957 are automatically exempt. Students can also apply for a medical, religious, or philosophical exemption from the Arkansas Department of Health. Forms may be requested by e-mailing: ( The number to call for questions is 501-661- 2169. This process must be done annually before the fall semester and the “letter of exemption” must be sent to JBU Health Services each year. There is a good deal of paperwork associated with these exemptions. Those filing for an exemption should ask the state to issue a letter stating that they are in process of filing for an exemption so that JBU can have that in the student records until the exemption is official. Students who do not follow through with providing acceptable documentation of MMR immunization records or exemption will have a hold placed on their account and will be unable to register for courses for future semesters until the records are received and approved.
