Please have the captain for each *team fill out the following form:

*Note: Roster forms will be sent to the captain after this form has been submitted. If you do not get a response within 48 hours after submitting this form, please contact the IM office. All spaces are required. All rosters will be due on the day of the captain's meeting unless otherwise noted.

*Registration due by noon on deadline day.  No exceptions.
